Q: How do I apply for services?
A: New consumers need to complete the screening process with their local management entity screening, triage & referral department by phone or in person.  For assistance, contact your nearest Superior Healthcare Services site.
  

Q: I'm currently receiving services with another provider. How do I change providers?
A: First locate a new provider, then notify your current provider of your intent to switch providers and attend any discharge planning meeting, then sign appropriate consent forms given permission for your files to be transferred.


Q: What is a Diagnostic Assessment?
A: It is a comprehensive Mental Health and/or Substance Abuse exam conducted by two professionals from two disciplines. Typically conducted by a Therapist and a medical practitioner.  This exam determines eligibility for enhanced benefit services and treatment recommendations.


Q: What do I do if I want to refer someone to Superior Healthcare Services
A: You can download the screening/referral form from our website and fax or email it to the nearest SHSI office. Or you can call or come in person and we can take the referral information down for you.


Q: What is a PCP?
A: A PCP (Person Centered Plan) is a blueprint or road map to services being utilized and/or recommended for the consumer and family to assist them in regaining balance in their lives.  It specifies what services are to be rendered, recommendations, assessments needed and interventions that will be used.  It also contains the Crisis Plan to outline steps to prevent and react to a crisis situation.  The plan is consumer driven--meaning it is directed by the consumer's strengths, preferences and desires.
